Locality: U.S.A., Washington, King, Mount Baker-Snoqualmie National Forest, Old Cascade Highway area Forest Service Road 6099. From Highway 2 near Scenic take Old Highway north for 1.5 miles to end of road. Cross Tye River on foot bridge. Walked old road.

Habitat: Coniferous forest with Abies amabilis, Salmonberry and Hemlock. On bark very large Hemlock below road by 25 feet.
Reproductive Condition: apothecia
Notes: Apothecia 0.7mm, stalk green brown, head dark green, spores weak 1 septate ellipsoid 6 to 10. See EB #L5126 same tree. See EB #L5127A and L5127B same collection. Substrate: corticolous.
